PATIENT CODE OF CONDUCT

We are committed to providing a safe and healthy environment for staff, visitors, patients and their families.

Visitors, patients and accompanying family members are expected to refrain from behaviors that are disruptive or pose a threat to the rights or safety of other patients and staff. Adults are expected to supervise any children in their care and are responsible for any damages may they cause. 

If you witness or a subjected to inappropriate behavior by another visitor, please report to any staff member.

Violators are subject to immediate removal from the clinic and/or discharge from the practice. A list of inappropriate behaviours can be found in the ZERO-TOLERACNCE section of this page below.


CONFLICT RESOLUTION

As a patient visiting our practice, we expect you to take a direct and constructive approach to conflict resolution.

If you have any questions about your care or you are unhappy with the service received in our office, please contact our clinic manager to address your questions and concerns. manager@cityedge.ca

Questions about fees for services not covered by OHIP should also be addressed to manager@cityedge.ca


ZERO-TOLERANCE BEHAVIOURS

Violators are subject to immediate removal from the clinic and/or discharge from the practice.

  1. Intimidating or harassing staff or other patients

  2. Making threats of violence through phone calls, letters, voicemail, email or other forms of written, verbal or electronic communication.

  3. Physically assaulting or threatening to inflict bodily harm

  4. Making verbal threats to harm another individual or destroy property

  5. Damaging business equipment or property, including theft

  6. Making menacing or derogatory gestures

  7. Making comments that are or can be perceived as racist, sexist, homophobic, transphobic, ablest, ageist, or are derogatory in nature

  8. Possessing firearms or any weapon

  9. Refusing to leave when requested by a staff member
