CHANGE YOUR APPOINTMENT

Please notify us by phone or email as soon as possible if you are unable to attend your appointment for any reason. We will be happy to reschedule you.

LATE ARRIVAL

If you are running late, please call ahead to ensure we can still accomodate you when you arrive.

FEES FOR MISSED APPOINTMENT

MISSED PHYSICIAN APPOINTMENT FEE: $70

MISSED DIAGONISTIC TESTING APPOINTMENT: $35-$70

We are a specialist office. Missed appointments can deny other patients access to time sensitive vision care.

Cancellation notice must be provided a minimum of 24 hours in advance.

You will be required to pay any outstanding fees prior to being scheduled for another appointment. Patients who repeatedly miss appointments will be subject to dismissal from the practice.

If you are a new patient who fails to show for your initial appointment on more than once occasion, we will contact your referring provider to inform them of the lapse in your care. If this occurs, you will need a new referral to see our specialists.
